At Destiny Caregivers, we have a team of healthcare professionals who will assess and evaluate our patients’ needs. They follow a family-centered, proactive approach to care to ensure optimum health outcomes. A care plan shall be developed by our care coordinators with the help of patients, the patients’ families, and other healthcare providers. This care plan shall be implemented and modified as per the physician’s advice and recorded changes/improvements of the patient’s condition.
